Full TI-Pauschale with PTV5 update

From July 2023, medical practices and pharmacies will receive a fixed monthly lump sum to compensate for the equipment and operating costs of the telematics infrastructure instead of the previous one-off payment. There are conditions that must be observed for full payment.

Calculation of the TI-Pauschale for practices

The calculation of the TI-Pauschale for medical practices is based on the size of the practice, the time of initial equipment and the time of connector replacement.

Calculation of the TI-Pauschale for pharmacies

The calculation of the TI-Pauschale for pharmacies is based on the size of the pharmacy (two or more HBAs), the initial equipment (staggered according to the annual Rx fee) and the time of commissioning.

Basic requirement for the TI-Pauschale:
Medical practices and pharmacies must have installed the TI applications required by law. If one TI application is missing, the flat rate is reduced by 50%; if several are missing, the flat rate is reduced by 100%.

The TI applications in doctors' surgeries required by the Federal Ministry of Health (BMG) are

  • since 2023:
    • Emergency data management (NFDM) / electronic medication plan (eMP)
    • Electronic patient file (ePA)
    • Communication in the medical sector (KIM)
  • from 2024:
    • Electronic certificate of incapacity for work (eAU)
    • Electronic doctor's letter (eArztbrief)
    • Electronic prescriptions (ePrescription)

The software in pharmacies must support

  • the electronic patient record (ePA),
  • the electronic medication plan (eMP),
  • the e-prescription and
  • KIM (communication in medicine) by April 1, 2024 at the latest.

The term extension via software update (PTV5) includes all required TI applications. This means you receive the full TI-Pauschale and remain relaxed and securely connected until 2025.

Background

Payers are legally obliged to reimburse the necessary costs for the TI. Since July 1, 2023, this obligation has been implemented on the basis of monthly flat rates. The total of the ongoing operating costs and the pro rata investment costs in practices and pharmacies are extrapolated over a period of five years and then reimbursed in monthly lump sums.
